Certificates Required:

1. Age Certificate of the deceased.
2. Death Certificate .
3. Certificate issued by Talathi / Gramsevak / Tahasildar.
4. certifying deceased was Head of the family.
5. Income certificate issued by Talathi / Gramsevak / Tahasildar

Beneficiary:

Head of the family expires his widow.
